This item requires a valid prescription on file before your entire order can be shipped to you. In order to ship your order please include your doctor's information (doctor name phone and fax number, address patient name if other than yourself) along with your order in the 'Orders Comment' box on the Order Summary page.

If you already have a prescription and have not already done so, you may fax it to (877) 484-7091 along with your order number to expedite the shipment of your order.
