In order to ship your order please include your doctor's information (doctor name, phone and fax number, address, patient name if other than yourself) along with your order in the 'Orders comments' box on the bottom right of the next page.
If you already have a prescription and have not already done so, you may fax it to us at (877) 484-7091. If faxing your prescription please include your order number as this will help expedite the shipping of your order.
